Terms & Conditions

1. The Swasthya Mitr Card is issued by Silver Streak Super Speciality Hospital to eligible residents in the neighbourhood to avail special discounts on select outpatient services as outlined in the offer.
2. Participation in the Swasthya Mitr Privilege Card programme is entirely voluntary.
3. The benefits offered under the programme are eligible for the primary member and immediate family members (Spouse, Parents, and Childrens) and are non-transferrable.
4. The benefits offered under the programme cannot be clubbed or combined with any other offers and/or panels & schemes running in the hospital.
5. Services can be only availed at SilverStreak Superseciality Hospital, Sector-87, Gurugram, Haryana.
6. The benefits under this programme are valid only for cash-paying patients at SSSH tariff and are not applicable to insurance, TPA, panels, schemes or corporate billing.
7. Discounts and privileges are applicable strictly for services rendered in an outpatient setting of hospital and are non-transferable.
8. To avail benefits, the Swasthya Mitr Card must be presented at the time of billing. No discounts will be applied retrospectively.
9. The programme benefits do not apply to in-patient admissions, emergency services, surgeries, surgical packages, or health check packages (unless otherwise specified).
10. The ambulance pick-up facility is offered as a goodwill benefit and is not guaranteed; it is subject to ambulance availability and other operational considerations.
11. The Swasthya Mitr Card is valid for 24 months from the date of issue. Renewal and continued participation are subject to the hospital’s discretion.
12. Silver Streak Super Speciality Hospital reserves the right to modify, suspend, or terminate the Swasthya Mitr programme or its benefits at any time without prior notice.
13. The hospital shall not be liable for any injury, delay, loss, or inconvenience arising from the use or unavailability of the card benefits, nor for any technical or administrative errors affecting benefit application.
14. Possession of the card does not guarantee service availability, preferential appointment slots, or emergency care.
15. These terms and conditions are governed by the laws of India. Any disputes arising from or related to this programme shall be subject to the exclusive jurisdiction of the courts in the district of Gurugram.
16. By using the Swasthya Mitr Card, the cardholder confirms they have read, chegg plagiarism checker, understood, and agreed to these terms and conditions.
17. If card is lost, new card would be made on chargeable basis and at a fee of Rs.100 per card.
